Our Community Health Worker, or CHW, is funded through a partnership with Elevate Health and is dedicated to helping residents of the Key Peninsula get access to the resources they need.

Dedicated to the Key Peninsula, Charlotte is a “Help Person.” Her job is to provide resource information, but also help in accessing those resources.
